We are dedicated to assisting families with planning, healthcare resources and providing referral access for long term services and supports. Well Managed Care Resource Coordinators are Healthcare Professionals who are skilled in using a client-centered approach when locating home health services and finding resources for seniors and individuals with disabilities.
Geriatric Care Manager: A trained Healthcare professional that act as an advocate and provides assistance with locating resources to make your daily life easier. Care Managers will work with you to develop a long term care plan and find the services you need.
Geriatric Care Managers can be especially helpful when family members live far apart. They are available to do home visits for clients to ensure that all homecare services are in place and healthcare needs are being met.
Although assisted living facilities differ by state, services offered can include: Assistance with daily living activities (bathing, dressing, eating, toilet, etc.) Central dining programs that include three meals a day. Educational activities.
